LPN (2nd Shift) | Adult Forensic Residential Program

Job Purpose:

• The Licensed Practical Nurse (LPN) at LIFESTREAM BEHAVIORAL CENTER is responsible for providing high-quality nursing care to patients within the behavioral health setting. This role involves working under the supervision of registered nurses and physicians to ensure the delivery of compassionate and effective patient care during Shift 1.

Key Responsibilities

• Administer medications and treatments as prescribed by physicians and in accordance with nursing standards and protocols.
• Monitor and record patient vital signs, symptoms, and medical histories, and report any significant changes to the supervising nurse or physician.
• Assist in the development and implementation of patient care plans, ensuring they are tailored to meet the individual needs of each patient.
• Provide emotional support and education to patients and their families, helping them understand treatment plans and healthcare procedures.
• Maintain accurate and detailed patient records, ensuring confidentiality and compliance with healthcare regulations.
• Collaborate with a multidisciplinary team to coordinate patient care and ensure a holistic approach to treatment.
• Participate in ongoing training and professional development opportunities to enhance nursing skills and knowledge in the field of behavioral health.
• Ensure a safe and therapeutic environment for patients, adhering to all safety protocols and procedures.
• Assist with the admission, transfer, and discharge processes of patients, ensuring a smooth transition and continuity of care.
